If you have a Life-Threatening Food Allergy, this form must be completed and returned to a member of the Event Coordination Committee before Graduation night, 2019.  If you will be 18 year of age by June 11, 2019 (Graduation Night), you'll need to complete the form and sign yourself.  If you are still under the age of 18 by June 11, 2019, your Parent/Guardian must complete and sign the form for you.
