John “Jack” A. Perry, Jr. of Itasca, Illinois. 

Family and friends will gather for visitation for John “Jack” A. Perry, Jr., 56 will be held Wednesday, April 3 from 4:00-8:00 pm at Morizzo Family Funeral Home, 2550 Hassell Rd., Hoffman Estates, Illinois, 60169.  Services will be Thursday, April, 4 at 11:00 am for mass at St. Matthew Catholic Church, 1001 E. Schaumburg Rd., Schaumburg, Illinois; proceeding to interment at Queen of Heaven Cemetery, Hillside, Illinois. 

Born May 29, 1956 in Chicago to the late John, Sr. and the late Shirley (nee Koenen), he passed away suddenly March 27, 2013 at his home in Chicago. Jack was the loving brother of James (Andrea), Robert (Sandra), Sandra (Bradley) Semcek and William (Linda); fond uncle of Cheryl, Shannon, Shelly, Cody, Mark, Bobby, Alyssa, Bill and Nick; and proud great uncle of Gianna; dear nephew of Louise Rodgers, Patricia Luby, Mary Giaimo and Lois Arden dear cousin to many; he will be missed by his dear friends Shelia Washington, Sharyn Chrysokos, Bill Freund, Tom Grasso and many more.
