Carla J. Madej of Hoffman Estates A memorial Mass for Carla J. Madej (nee Werner), 56, formerly of Medinah, will be held on Friday, Nov. 16, at 10 a.m. at St. Theresa Catholic Church, 455 N. Benton St., Palatine. Interment will be private. Carla was born on June 18, 1956, in Evergreen Park, the daughter of the late Carl and Emily Werner, and passed away on Oct. 26, 2012, at Alexian Brothers in Elk Grove Village. Carla had worked as director of human resources for Orchard First Source of Rolling Meadows. She was the beloved wife of Terrence Madej; and loving sister of William (Linda) Werner and Deb (Chuck) Werner-Johnson. In lieu of flowers, memorials in memory of Carla would be appreciated for the American Cancer Society, 1114 N. Arlington Heights Road, Arlington Heights, IL 60004.
