MADISON - Holbrook, Charles R. "Chuck" Jr., 55, passed away Saturday, April 19, 2014. 

He was a forklift operator with Madison Precision Products. 

He was preceded in death by his father, Charles Holbrook Sr. and the love of his life, Rhonda Mitchell. 

He is survived by his son, Jared Holbrook, mother, Carolyn Holbrook and his brother, Jeff Holbrook (Nelsonya). 

Funeral services will be 11 a.m. Thursday at Fairdale-McDaniel Funeral Home 411 Fairdale Road, Fairdale, KY. Burial to follow. Visitation will be 2-8 p.m. Wednesday at the funeral home.
