Maybe my 15 minutes of fame came in 1978 when I was a manager for the Nuclear Regulatory Commission in the King of Prussia office. I got a routine report from the Three Mile Island 2 plant about a valve that failed open during a pre- operational test causing a loss of coolant event before they had put nuclear fuel in the reactor. I wrote a urgent memo to my management requesting an assessment and corrective action by another part of the NRC. It was turned down. One year later plus a day, the same thing happened during operation with fuel. This time the valve failure resulted in the core melting and a release of radioactive material from the plant. Committees from Congress, the President and the Chairman of the NRC all deposed me and suggested if I had been more aggressive I could have prevented the accident. When I asked if the problem was with my upper management who ignored my warning and request, they told me they didn't have time to depose my managers to understand why they didn't do anything.
