ANDREW D. KOOISTRA, of Fort Wayne, died on Monday, April 22, 2013. Born Feb. 22, 1950, in Fresno, Calif., he was a son of the late John Kooistra and Leota (Countryman). He was a material management tech at St. Joseph Medical Center. Surviving are his brother, Kenneth M. (Susan) Kooistra of Schwenksville, Pa. and nieces, Kathryn Kooistra of Phoenixville, Pa. and Kristin (James) Kisinger of Ulster, Pa.
