Welcome to the 40 Year Class Reunion Registration Form. We have two forms of payments available for your convenience - check or credit card. If paying by credit card there is a 4.9% processing fee that is already figured into the total cost of the dinner ticket, class picture and class booklet. However, if you are paying by check there is no processing fee which you will see when you click payment by check. We do have a deadline date for registration and payment of JUNE 1, 2012. NO REFUND will be made after JUNE 1, 2012.
