Jack passed away on November 11, 2012 from an apparent heart attack.  Late in life, he had several health issues including several heart stents and renal failure.  Born May 8, 1952, he was 60 years of age.

Jack had a great time at the 40th class reunion in 2010.  He seemed elated to be able to see all his friends and mentioned several times he just might move back home.  I'm sure everyone would have loved to see him do that but he couldn't get away from his doctors in California.
