Joe,
We are in NC. Not sure how it works in SC. Here you can sign up online through your medical provider portal. That is how we did it at first through Novant. Then we learned we could sign up at Atrum as well even though we did not have an association with them. That was a phone call (see number below) and someone spoke with us on the first attempt and scheduled us for February. We kept signing onto Novant and bingo saw the appointment for the 25th. We were told by neighbors, there are cancellations when there is inclement weather so it does not hurt to keep checking and reschedule for an earlier appointment.
Russell's appointment yesterday in Matthews (Novant) went well. We arrived in the rain and there was a line waiting outside. All total, it took about an hour for the whole process. He feels fine with no side effects so far. They scheduled his 2nd shot on Feb. 15th. He has to wait 21 days with the Pfizer vaccine.
This morning I went on Novant MyChart portal and slew of appointments had opened up in Matthews for tomorrow. SO I set a 7:45 am appointment. The sooner you can get the vaccine the better I feel. I will contact Atrium today and cancel our Feb. 18th appointments so someone else can take those slots.
Below is the information our neighbor sent us to register through Atrium. You can call 704-468-8888.

As far as consent forms, we agreed to those online through echeckin. They screen you when you show up for the appointment. With Novant, you go inside for processing and then the shot is administered in a private room. Keep in mind Atrium we were told is a drive by appointment so not sure how the screening works from our car. Russell was given a wallet size vaccine card when he left.
