Simply answer the questions and send.  Though we STRONGLY encourage registering online, a pdf file is available upon request at shakerclass69@gmail.com. 

The full weekend includes Friday night, Saturday panels/lunch/memorial service, Saturday night and Sunday brunch. If by some chance your guest wants to also attend the Saturday panel/lunch, we will accommodate an extra lunch at no charge. As with previous reunions, there is no "Saturday night only" option. 

Reunion costs are:

- Full weekend for one - $130
- Full weekend for two - $230
- Friday night classmate reception only - $35
- Saturday panels/lunch only - $15
- Friday night reception and Saturday panels/lunch - $50
- Sunday brunch only (no charge)

You can make your payment online using Venmo or by check.**

If paying by Venmo, please indicate Debbie (Morris) Melnyk as the recipient (debnadzam@gmail.com).  Debbie will transfer the Venmo payment directly into the Shaker 69 Reunion PNC checking account.

If paying by check, please indicate to whom you have sent your payment check. It needs to be payable to either Ron Leikin at 38750 Mentor Avenue, Willoughby, OH 44094 or Debbie Melnyk at 26143 Rose Road, Westlake, OH 44145. In the memo line please indicate "Shaker Reunion Payment". 

**Please note the payment options and addresses prior to closing out.  Once you exit the survey you cannot get back into your responses.
