Starlet Forms & Infomation

The following forms will need to be completed in their entirety and turned into Ms. Lisa or Ms. Joy by or on the 1st day of Band Camp.  If you do not have the forms complete and insurance purchased, you will not be allowed on the field until this is taken care of.  The forms and insurance are required by Hillsborough County:

School Insurance can be purchased at their website:

You will want to purchase the following insurance.  This does not act as your primary insurance, only as a back up.  As we found out last year, it is consdiered 3rd party insurance and only pays after your primary insurance has been billed.   If you would rather, you can pay the Starlets $20.00 and I will order your insurance for you.  Either way, I will need a copy of the insurance card once your have purchased the insurance.

 

Group D High School Non-Contact FHSAA Sports

Provides basic accident insurance for FHSAA Cheerleading, Golf, Cross Country, Track, Swimming, Girls Flag Football, Tennis, Marching Band, Auxiliary Units, and Team Trainers/Managers during school sponsored practices and competition during the regular 2013-2014 school term, beginning in August. Also provides coverage for conditioning while on school premises and while under the direct supervision of a school coach. No coverage is provided for the sports listed in groups A, B or C.
